Job summary

CMHA Durham is seeking a Nurse Practitioner to provide comprehensive primary health care and health promotion across the lifespan within our Durham Region community.

You will work with a multidisciplinary team to assess, diagnose, and manage care, emphasizing prevention, education, and person-centered planning. Strong collaboration, documentation, and community health initiative experience are essential.

Qualifications

  • Bachelor of Science in Nursing and active RN(EC) registration in Ontario.
  • Master's degree in nursing or health-related field preferred.
  • Experience with electronic medical records and drug databases is an asset.

Responsibilities

  • Perform comprehensive health assessments within RN(EC) scope of practice.
  • Collaborate with physicians and other health professionals to develop patient care plans.
  • Provide health education, disease prevention, and risk factor modification.
  • Document visits accurately and maintain client health records.
  • Deliver nursing care and pharmacological or counselling interventions as needed.
  • Support clients and families in decision-making and self-management.
  • Identify community needs and contribute to program development.
