Job summary

Waypoint Centre for Mental Health Care in Ontario seeks a Nurse Practitioner to join the Integrated Ambulatory Mental Health team. The role focuses on primary mental health care within a step-down model, patient monitoring, treatment coordination, and program development in collaboration with the Medical Affairs department.

The position offers hybrid in-office and remote work and a competitive rate of $65.31–$71.07 per hour.

Qualifications

• Master's degree in nursing or related graduate degree with clinical focus required.
• Completion of a recognized Nurse Practitioner program required.
• Current registration as an RN in the Extended Class with the College of Nurses of Ontario required.

Responsibilities

• Provide primary mental health care within a step-down model with ongoing monitoring and care coordination.
• Support patients on long-acting injectable medications and clozapine with monitoring and testing as needed.
• Lead development, implementation, and evaluation of clinical protocols and pathways with the Integrated Ambulatory team.
• Ensure timely access, efficient care processes, and continuity across patient treatment.
• Collaborate with psychiatry and the Medical Director to maintain continuity of care and clinician mentorship.
• Document assessments, treatment plans, and follow-up needs accurately to support continuity of care.
