Job summary

Ontario Health atHome seeks an experienced Nurse Practitioner (NP) specializing in hospice palliative care to lead clinical care for patients at home and support primary care providers within the PCOT network.

You will participate in education, collaborate with a broad team of health professionals, and help advance compassionate, patient-centered care across Ontario Health atHome in community, office, and facility settings.

Qualifications

• RN(EC) registration with College of Nurses of Ontario.
• Master’s degree preferred.
• Canadian Nursing Association Certification in Hospice Palliative Care preferred.

Responsibilities

• Provide comprehensive health care services including pain and symptom management per CNO Standards.
• Be an integral member of the Palliative Care Outreach Team (PCOT).
• Participate in continuing education and mentoring within Ontario Health atHome.
• Anticipate changing priorities and adjust to meet the needs of patients and families.
• Foster collaboration with palliative care system partners and adhere to safety policies.
