Job summary

BlueSky Personnel Solutions is seeking a bilingual Nurse Practitioner (French/English) to join a Hamilton primary care team, onsite. You will provide comprehensive health care focused on promotion, illness prevention, and chronic disease management, working with physicians and other professionals to deliver high-quality community care.

You will work both independently and with an interdisciplinary team. The role offers a competitive salary (114,000–138,000 CAD) and benefits, with standard hours

Qualifications

• BSN and NP certificate from a recognized university.
• Good standing with the College of Nurses of Ontario (CNO).
• Excellent clinical judgment and communication.
• Ability to work independently and within a multidisciplinary team.
• Proficient in standard computer applications and electronic health records.

Responsibilities

• Provide primary health care services to clients of all ages.
• Assess, diagnose, and manage common health conditions independently or with physician support.
• Engage clients in identifying health risks, setting goals, and choosing care plans.
• Prescribe and interpret diagnostic tests as needed.
• Maintain accurate, confidential client health records.
• Support clients with stable chronic conditions and ensure appropriate follow-up care.
• Refer clients to internal programs and community resources.
• Participate in policy and procedure development, quality improvement, and data collection.
• Collaborate with an interdisciplinary team to ensure coordinated, holistic care.

Location: Hamilton - Onsite

Salary range: $114.000 to $138.000

Monday to Friday - Flexible working hours between 8:00am - 5:00pm

Our client is seeking a Bilingual Nurse Practitioner (French/English) to join their primary care team in Hamilton. Reporting to the Primary Care Services Coordinator, you'll provide comprehensive, client-centered health care focused on health promotion, illness prevention, and chronic disease management.

You'll work both independently and collaboratively with physicians and other healthcare professionals to deliver quality care that supports the well-being of our community.
