AgeCare Willowgrove in Ancaster, Ontario is seeking a part-time Nurse Practitioner to collaborate with the MD and interdisciplinary team to deliver resident care in a long-term care setting. You will assess and manage acute and chronic conditions, prescribe treatments, and participate in care conferences, with leadership support for the LTC team.

The role requires at least one year of geriatric/long-term care nursing experience, Ontario RN Extended Class registration, and knowledge of applicable

Qualifications

• Minimum one year clinical experience in geriatric, rehabilitation or long-term care nursing.
• Leadership knowledge/experience is asset.
• Certifications in gerontology, mental health, behavioral support or palliative care are assets.
• Current registration with the College of Nurses of Ontario as a Registered Nurse in the Extended Class (PHC or Adult).
• Familiarity with Long Term Care Act and related standards is an asset.

Responsibilities

• Collaborate with MD, DOC and interdisciplinary team to deliver resident care.
• Assess, diagnose, and evaluate acute and chronic illnesses and follow up care.
• Support residents and SDMs in care decisions and advocate for best interests.
• Provide leadership within the LTC team on move-in/out, discharge planning, and palliative care.
