Job summary

AgeCare Willowgrove in Ancaster, Ontario is seeking a Part-Time Nurse Practitioner to support resident care within a geriatric long-term care setting. You will work collaboratively with the MD, DOC and interdisciplinary team to assess and manage acute and chronic conditions and to plan follow-up care.

The role emphasizes caregiver support, resident advocacy, and leadership within the LTC team, including move-ins/outs, discharge planning, palliative and dementia care.

Qualifications

• Minimum one year clinical and/or working experience in geriatric, rehabilitation, or long-term care nursing.
• Leadership knowledge/experience is an asset.
• Certifications in gerontology, mental health, behavioral support, or palliative care are assets.
• Current registration with the College of Nurses of Ontario as an RN in the Extended Class (PHC or Adult) and entitled to practice.

Responsibilities

• Collaborate with the MD, DOC and interdisciplinary team in care delivery.
• Assess, diagnose, and manage acute and chronic conditions; prescribe treatments and follow-up.
• Support residents and families; involve SDM in care decisions.
• Participate in resident care conferences and planning.
