About the Opportunity :

An established long-term care facility in Toronto is seeking a dedicated Nurse Practitioner (NP) to join its clinical leadership team. This is a hands-on, high-impact role focused on delivering primary care to residents while also supporting research, education, and quality improvement initiatives.

This opportunity is ideal for a compassionate and experienced NP who thrives in interdisciplinary teams, values person-centered care, and is passionate about improving outcomes in senior care settings.

What You’ll Be Doing :

Resident Care (70–80%)

  • Deliver comprehensive primary care to residents with acute and chronic conditions.
  • Conduct detailed health assessments and physical exams; order and interpret diagnostic tests.
  • Develop care plans in partnership with residents, families, and team members.
  • Manage hospital discharges, palliative and end-of-life care, pain and dementia care.
  • Apply knowledge of diverse health conditions and social determinants to support well-rounded care.
  • Act as a liaison with hospitals and health partners to ensure continuity of care.

Leadership, Research & Education (20–30%)

  • Participate in interdisciplinary research projects and quality improvement initiatives.
  • Mentor and educate clinical staff, students, and other healthcare professionals.
  • Support the adoption of best practices and compliance with long-term care regulations.
  • Represent the care team on internal committees and professional advisory groups.

What You Bring :

  • Nurse Practitioner registration with the College of Nurses of Ontario (Extended Class).
  • BScN required; MScN or MN preferred.
  • Membership with NPAO and liability insurance (e.g., through RNAO) preferred.
  • Certification in gerontology is an asset.
  • Previous experience in long-term care or geriatric settings strongly preferred.
  • Strong clinical reasoning and communication skills.
  • A people-first mindset with the ability to navigate complex or high-needs resident populations.
  • Flexibility to provide after-hours or on-call coverage as needed.
  • This is an independent contractor position – no benefits or paid time off included.
  • On-site work required in a central Toronto long-term care facility.
  • While full-time is preferred, qualified part-time applicants are welcome to apply.
