Job summary

Guelph Community Health Centre in Ontario is seeking a Nurse Practitioner to join an interdisciplinary team delivering specialty care focused on HIV, gender-affirming services and other priority populations. The NP conducts comprehensive assessments and coordinates care.

Responsibilities include diagnosis, treatment, counseling, education, maintaining electronic medical records, and collaboration with physicians and other providers.

Qualifications

• Registration in the Extended Class with the College of Nurses of Ontario (CNO).
• Experience providing medical care to people living with HIV, at-risk populations, and gender-affirming care seekers.
• Experience in community health or social services is an asset.
• Excellent communication skills and ability to prioritize and multi-task.

Responsibilities

• Provide ongoing, specialty health care within priority populations (HIV, at-risk individuals, gender-affirming care).
• Conduct comprehensive assessments, diagnosis, counseling, screening, referrals, education, treatment and follow-up.
• Determine health problems considering lifestyle and psychosocial factors; maintain electronic medical records.
• Collaborate within an interdisciplinary team and participate in chart reviews and client inquiries.
• Note: Nurse Practitioner may be required to share on-call duties with other providers in the future.

Working as part of an interdisciplinary team to provide clinical health services for people living with and at risk of HIV and people seeking gender-affirming care, the Nurse Practitioner (NP)provides specialized health care with a strong emphasis on health promotion and disease prevention within the scope of practice and standards of care outlined in the Expanded Nursing Services for Patients Act and Regulations and the Practice Expectations Specific to Registered Nurses in the Extended Class (NPs). The NP diagnoses and manages common health problems and diseases both independently and in collaboration with the client and the physician. The Nurse Practitioner works as part of an interdisciplinary team to address issues in the community that affect health. They are responsible and accountable for their practice.

• Provide ongoing, comprehensive specialty health care to clients within priority population groups, specifically people living with HIV, people at-risk of HIV and people seeking Gender-Affirming Care, including addressing the health care needs of transgender clients and providing transition-related care.
• Provide comprehensive assessments, diagnosis, counselling, screening, referral, education, treatment and follow-up, supportive client counseling, and crisis intervention.
• Determine the presence of existing and potential health problems, with a focus on lifestyle, psychological,socioculturaland environmental risk factors that may influence the client's health status
• Maintaincomplete,accurate, andtimelyelectronic medical records.
• Participate in chart reviews, discussion of problem cases, and client inquiries
• Work within adeterminant of health framework which emphasizes health promotion and education, disease prevention and recognizes the special risk factors which may affect the health of clients
• Registration in the Extended Class with the College of Nurses of Ontario
• Member of the Nurse Practitioners Association of Ontario
• Experience working with communities and individuals who face barriers to accessing health care due illness and other factors known to be the determinants of health (poverty, education, language barriers, discriminations, mentalhealthand substance use issues, etc.).
• Experience providing medical care for people who are HIV positive, people at-risk of HIV and people seeking gender-affirming care.
• Previousexperience in community health/social services and/or a values-based organization is an asset
• Able to independently manage complex clients with comorbidities and vulnerabilities, consulting as appropriate
• Excellent communications skills, both verbal and written, along with the ability to prioritize and multi-task.
• Proficient in the use of computers and various other software applications including Word, Outlook,PSSand electronic health records
