Job summary

SOAHAC in Owen Sound, Ontario seeks a Nurse Practitioner to provide primary care across the lifespan within an interdisciplinary team. You will work with physicians, traditional healers, and community partners to deliver accessible, culturally safe care to Indigenous clients.

The role requires BScN and CNO registration as RN in the Extended Class, plus a Primary Health Care NP certificate. Experience in primary care and cultural knowledge of Southwestern Ontario communities is preferred.

Qualifications

• BScN with current registration as Registered Nurse in the Extended Class (RECOGNIZED by CNO)
• Primary Health Care NP Certificate
• Experience in primary care, chronic disease management, mental health or addictions is an asset
• Knowledge of Indigenous communities in Southwestern Ontario
• Strong organizational and communication skills
• Proficiency with EMR, Word, Excel, PowerPoint
• Willingness to participate in Indigenous teachings and ceremonies

Responsibilities

• Provide comprehensive primary care across lifespan for urban Indigenous community
• Collaborate with physicians, traditional healers, and allied health professionals
• Conduct on-site assessments, telephone and outreach visits
• Review lab results and coordinate follow-ups/referrals
• Maintain confidentiality and accurate documentation
• Support outreach and community partner activities

We are seeking a Nurse Practitioner (NP) to join our interdisciplinary team in Owen Sound, Ontario. The Nurse Practitioner provides primary care across the lifespan, supporting members of the urban Indigenous community and their families. You’ll work closely with physicians, traditional healers, community partners, and allied health professionals to deliver accessible, culturally safe, patient-centered care.

Requirements:

• BScN and current registration with the College of Nurses of Ontario as a Registered Nurse in the Extended Class
• Primary Health Care Nurse Practitioner Certificate
• Experience in primary care, chronic disease management, mental health, or addictions is an asset- new graduates are encouraged to apply
• Knowledge of culture and local Indigenous communities in Southwestern Ontario is required
• Strong organizational skills with the ability to work effectively and independently
• Strong attention to detail and assessment skills
• Ability to work effectively as part of an interdisciplinary team
• Ability to establish and maintain effective working relationships with others and across teams
• Excellent interpersonal skills with the ability to communicate clearly
• Computer literacy including skills in word processing, e-mail, PowerPoint, Excel, and Electronic Medical Records
• Willingness to participate in ongoing Indigenous teachings and ceremonies
• Valid Ontario Driver’s “G” license, personal auto insurance (at least 3 years experience) and clean driver’s abstract
• Obtain/ maintain a current and satisfactory police check as a condition of employment
• Up-to-date immunizations and records

Responsibilities:

• Provide comprehensive primary care services to clients of all ages, including primary prevention, health education, comprehensive physical exams, chronic disease management, antenatal care, and acute/episodic visits
• Conduct on site assessments, as well as telephone and outreach visits
• Advocate on behalf of clients with regards to equitable access to traditional healing and western health services
• Review client laboratory results, procedure reports and consultation notes
• Arrange appropriate follow up/referrals as required
• Document all client contact and ensure all client information is always kept confidential
• Maintain close collaborative working relationships with other allied health professionals
• Work in collaboration with other health professionals to ensure a broad range of services are available to clients
• Consult with physicians as needed regarding issues that are outside the NP scope of practice
• Collaborate with community partners to offer outreach services as required
