Job summary

Guelph Community Health Centre is seeking a registered nurse to join our interdisciplinary team serving Shelldale and Downtown Clinic locations.

The nurse will support complex and vulnerable clients, manage mental health and addiction care, and work independently at top of scope with primary care colleagues.

Qualifications

• Bachelor of Nursing required with active license in Ontario.
• 2–5 years of broad nursing experience in hospital and/or community settings.
• Experience with harm reduction, addictions and mental health care.

Responsibilities

• Follow up and collaboratively manage patients on mental health medications and opioid therapies.
• Screen, monitor and assess patients using depression, anxiety, chronic pain and addiction tools.
• Complete in-person and virtual primary care visits independently, in consultation with the primary care provider.
• Liaise with physicians and other health professionals; manage referrals and external care decisions.
• Participate in case conferences and develop care plans with allied health colleagues.
• Manage transgender transition-related care and aftercare support.
• Outreach nursing, including home visits.

The Guelph Community Health Centre (GCHC) is located in the vibrant city of Guelph, Ontario, and provides primary care services and community health programs using an interdisciplinary, team-based approach. We have innovative client-centered services delivered through the use of interdisciplinary teams. Our clinical team consists of family physicians, nurse practitioners, and registered practical nurses, who work collaboratively with GCHC community health workers, including social workers, dietitians, outreach workers, and cultural interpreters.

The GCHC works within a determinant of health framework which emphasizes health promotion and education, disease prevention, anti-oppression, and recognition of special risk factors (social, physical, psychological, etc.) which may affect the health of clients. We are looking for a nurse who is passionate about working with complex and vulnerable clients to deliver outstanding health care. Principal Job Duties:

Clinical:

• Follow up and collaboratively manage patients taking mental health medications, opioids and opioid replacement therapy.
• Screen, monitor and assess patients using appropriate depression, anxiety, chronic pain and addictions assessment tools.
• Complete in-person and virtual primary care visits independently, in consultation with the primary care provider. Visits may include but are not limited to complex chronic disease management and health teaching, pre-natal and childcare and episodic health care concerns.
• Liaise with physicians, other health professionals and community partners. Compose and manage specialist referrals and act as a point of contact for external health care providers consulting on care decisions.
• Participate in case conferences and develop care plans in collaboration with allied health colleagues.
• Manage care journey for patients undergoing transgender transition-related surgeries, including co-planning via independent visits with patients, referrals and form completion and aftercare support.
• Outreach nursing, including home visits.

Knowledge, Skills, and Abilities:

• Bachelor's degree in nursing.
• Current College of Nurses of Ontario Certificate of Competence.
• Two to five years of broad-based practical nursing experience in a combination of hospital and community settings and/ or public health. Experience working in community settings independently is an asset.
• Knowledge and experience working with harm reduction philosophies and practices.
• Experience with addictions, mental health and complex chronic disease management.
• Experience working in an outreach capacity.
• Experience and comfort working with vulnerable populations, complex mental health concerns and individuals who use substances.
• Demonstrated commitment to and knowledge of community-based health care.
• High level of computer proficiency including knowledge of electronic medical records. Experience using Practice Solutions Suite is an asset.
• Ability to set goals and get results within a set timeline.
• Ability to multitask, take initiative and work independently.
• Effective written and oral communication skills.
• Highly motivated with effective time management, decision making, and organizational skills.
• A good team member and supportive of interdisciplinary practice.
• High level of confidentiality.
• Flexible towards change.
• Valid G license.

Working Environment:

• Evening and weekend work may be required

• 70% client/ 1:1 interaction

• 30% desk, meetings, frequent change of position

• Work may be performed at any GCHC location

• Possible exposure to clients in escalated or crisis situations

• Interaction with people who struggle with mental health or addiction issues, persons who are unhoused or may be involved in the criminal justice system
