Job summary

The Northwest Territories Health and Social Services Authority (NTHSSA) is seeking a Clinical Social Worker and Team Lead for the Transitional Care Planning Team in Yellowknife. Qualifications

• Master's Degree in Social Work with hospital discharge planning experience.
• Bachelor's Degree in Social Work with 7 years hospital-based experience.
• Registration with the Territorial Association of Social Workers prior to employment.

Responsibilities

• Provide clinical leadership, supervision, mentorship, and support to the Transitional Care Planning Team.
• Lead complex discharge and transition planning for patients with mental health, medical, and social care needs.
• Deliver direct social work, counselling, assessment, and case management services to patients and families.
• Collaborate with physicians, nurses, allied health professionals, community agencies, Indigenous organizations, and government partners.
• Support implementation and ongoing requirements of the Northwest Territories Mental Health Act.
• Coordinate care and community treatment planning for high-risk and complex patients.
• Promote recovery-oriented, person-centered, and culturally safe care practices.
• Participate in quality improvement initiatives, staff development, recruitment, and program planning.

As the Clinical Social Worker and Team Lead, Transitional Care Planning Team, you will provide clinical leadership while working directly with patients experiencing mental health, addictions, medical, and social challenges. You will lead a multidisciplinary team focused on discharge planning, case management, and ensuring patients receive the services and supports they need when transitioning from hospital to community care.

Key Responsibilities:

• -Provide clinical leadership, supervision, mentorship, and support to the Transitional Care Planning Team.
• -Lead complex discharge and transition planning for patients with mental health, medical, and social care needs.
• -Deliver direct social work, counselling, assessment, and case management services to patients and families.
• -Collaborate with physicians, nurses, allied health professionals, community agencies, Indigenous organizations, and government partners.

Key Requirements:

• -A Master's Degree in Social Work; and
• -At least 5 years of progressively responsible social work/discharge planning experience in a hospital setting;
• OR
• -A Bachelor's Degree in Social Work;
• -At least 7 years of related hospital-based social work/discharge planning experience.

Additional Requirements:

• -Registration with the applicable Territorial Association of Social Workers prior to employment
