Job summary

Southlake Health is seeking a Nurse Coordinator to lead coordination, delivery, and ongoing management of the Southlake@home programs. You will work with patients, families, physicians, and community providers to assess eligibility, coordinate discharge planning, and ensure safe transitions from hospital to home.

This role emphasizes integrated, population-health focused care, with a focus on reducing hospital days and ALD pressures through effective transitions and collaboration with OH@home

Qualifications

• Degree in a regulated health profession (BScN).
• Current registration with the appropriate regulating college.
• Two years’ experience in care coordination, advocacy and discharge planning in healthcare.
• Knowledge of community resources and applicable legislation.
• Excellent assessment, negotiation and problem-solving skills.
• Excellent interpersonal, communication, organization and time management.
• Bilingualism (English/French) is an asset.

Responsibilities

• Coordinate daily liaison with community providers and participate in joint problem solving.
• Provide case management to ensure provider interventions meet patient goals and facilitate transitions to CLHIN home and community care.
• Support patient flow and communication between hospital, patients, families and care partners.
• Contribute to quality improvement, program evaluation and expansion of Southlake@home.
