Job summary

Southlake Health is seeking a Case Manager to oversee a designated group of program participants, acting as a liaison between participants and their primary care physicians and other providers. You will perform ongoing assessments and help coordinate education and services with the care team.

The role emphasizes motivational support, coordination of cardiovascular rehab activities, and collaborative planning within an inter-professional team.

Qualifications

• Bachelor degree in kinesiology or related field with cardio/physio electives.

Responsibilities

• Case manage program participants and liaise with primary care physicians and other providers.
• Ongoing assessment of progress and adjust treatment with MD and team.
• Assist development, coordination and delivery of participant education.
• Provide motivational support for long-term behaviour changes.
