Job summary

The Saskatchewan Health Authority is seeking a Patient Flow Coordinator in Swift Current to provide a single point of contact for client discharges, transfers and admissions. You will coordinate bed utilization across IRH and SHA, using MDS as a screening tool for LTC eligibility and placement.

With a BScN and 5+ years in long term care or home care, you will lead data integrity efforts, support change management, and mentor staff while ensuring responsive, safe patient flow across the region.

Qualifications

  • Bachelor of Science in Nursing (BScN) required.
  • Minimum five years of current experience in long term care or home care.
  • CRNS registration or eligibility is required.

Job Description: The Patient Flow Coordinator is responsible for: Providing single point of contact for clients discharging within the SHA. Providing single point of contact for clients transfers, admissions and discharges with SHA. Coordinating the utilization of the electronic assessment system RAI-MDS (hereafter referred to as MDS) as the Screening tool for Long Term Care eligibility and placement and alternate level of care services placement in the region. Maintaining the electronic bed utilization system for IRH and the SHA. Maintaining the electronic bed utilization system for the region. Providing an effective program and continuous quality improvement and management practice to ensure efficient utilization of the system in consultation with Managers and Directors of Rural Health Services. Maintaining data integrity and data submission to the region as required.

Experience
  • Demonstrated experience with staff development and training.
  • Demonstrated experience in MDS systems and interpreting MDS outcomes.
  • A minimum of five (5) years current experience in long term care or home care
Education
  • Bachelor of Science in Nursing (BScN)
Licenses
  • Valid Class 5 driver’s license
  • Basic Life Support Certificate (BLS)
Other Education and Training
  • Psychiatric Nursing diploma is acceptable
  • Nursing diploma is acceptable
  • Bachelor of Psychiatric Nursing is acceptable
