Job summary

CarePartners is seeking a Nurse Specialized in Wound, Ostomy & Continence (NSWOC) to provide wound and ostomy consultation in clinic and community settings. You will promote patient self-management and lead the interdisciplinary team to achieve optimal outcomes.

The role covers the Waterloo-Wellington region with full nursing scope and a supportive orientation, ongoing clinical support, and access to scheduling tools and equipment.

Qualifications

• RN with NSWOC certification required, Ontario registration in good standing.
• Extensive wound and ostomy care experience and knowledge.
• Excellent English verbal and written communication.

Responsibilities

• Assess and manage wounds and ostomies in clinic and home settings.
• Promote patient self-management and coordinate with interdisciplinary team.
• Provide education and support to patients and families.
• Document care plans and outcomes, follow evidence-based guidelines.
• Travel within Waterloo-Wellington region for patient visits.
