Job summary

CarePartners is seeking a NSWOC-certified Registered Nurse to provide wound and ostomy consultation in clinic and community settings across the Waterloo-Wellington region. The role emphasizes promoting self-management and leading the interdisciplinary team to achieve optimal patient outcomes.

Qualifications

• Registered Nurse (RN) diploma and active CNO registration in good standing.
• Graduate of Nurses Specialized in Wound, Ostomy & Continence Canada (NSWOCC) certification.
• Experience in wounds and ostomies with nursing scope of practice.

Responsibilities

• Assess and manage wounds and ostomy care in clinic and home settings.
• Perform comprehensive wound assessments and apply evidence-based care.
• Promote patient self-management and engage interdisciplinary team for outcomes.
• Provide conservative sharp wound debridement (CSWD) and other debridement methods.
• Support patient safety following policy and standards.
