Job summary

Pacifica Housing seeks a Housing Support Worker to provide person-centered, housing-first services to tenants within its Supportive Housing programs. You will document progress, support case management, and coordinate with counselors, ACT teams and health professionals.

The role emphasizes life skills coaching, financial guidance, and advocating for tenants in on-site and off-site programming within a fast-paced nonprofit housing setting.

Qualifications

• Knowledge of the psychosocial rehabilitation model.
• Able to manage a fast-paced, high-pressure environment.
• Working knowledge of harm reduction and Housing First theory and practice.
• Proficient in MS Office programs including Word, Outlook and Excel.
• First Aid Certification is required.
• Naloxone administration training is required.

Responsibilities

• Provides individualized, person-centered support to tenants, including creating and evaluating personalized support plans.
• Assists with case management by identifying problems and reporting difficulties.
• Provides input to counsellors and other professionals for program development.
• Monitors tenant mental health and substance use, providing additional support as needed.
• Coordinates referrals to essential services and supports tenant engagement in community activities.
