Job summary

Pro Vita Care Management in Nanaimo is seeking a Licensed Practical Nurse (LPN) to join Wexford Creek Seniors Community. This rewarding role involves caring for residents and supporting a compassionate care team.

The position offers various full-time schedules (8am-8pm, 12-hour shifts) and temporary coverage through Oct 2026, with casual hours also available. Active LPN registration with BCCNM and current First Aid/CPR, as well as vaccination proof, are required.

Qualifications

• Graduation from a recognized Practical Nurses program or equivalent per BCCNM.
• Active registration as an LPN with BCCNM is required.
• Valid First Aid and CPR "A" certificate.
• Occupational First Aid Level 1 status (may be Level 2 depending on site).
• Satisfactory proof of COVID-19 vaccinations.

Responsibilities

• Observes, monitors, documents and updates residents' physical and psychological condition, care and nursing procedures.
• Provides and assists with personal care to residents as required.
• Supports emergency staff scheduling, as required.
• Ensures adherence to Infection Control Practices and provides education on outbreaks and infection control.

Available Positions

• Permanent Full-time: 3on 2off & 2on 3 off & 2on 2off, 8am - 8pm (12 hours shift)
• Permanent Full-time: 4 on 2 off 7am - 3pm
• Permanent Full-time: 4 on 2 off 7am - 3pm
• Temporary Full-time (until October 2026):3on 2off & 2on 3 off & 2on 2off, 8am - 8pm (12 hours shift)

Licensed Practical Nurse / LPN Must Haves:

• Graduation from a recognized Practical Nurses program or equivalent combination of education, training, and experience recognized by the BCCNM.
• Active registration as an LPN with BCCNM
• Maintains a Valid First Aid and CPR “A” Certificate.
• Maintain Occupational First Aide Level 1 status (may be a Level 2 depending on site).
• Satisfactory proof of COVID-19 vaccinations.
• Passes and complies with Ministry of Justice Criminal record Check upon hire and every 5 years thereafter.
• Ability to effectively communicate in English, both verbal and written.

Licensed Practical Nurse / LPN Responsibilities:

• Observes, monitors, documents and updates information about resident’s physical and psychological condition, care, and nursing procedures.
• Provides and assists with personal care to residents as required.
• Supports emergency staff scheduling, as required.
• Ensures adherence to Infection Control Practices and provides education as required for Outbreak and basic infection control practices.

Assesses resident health status including, but not limited to:

• Differentiating between normal and abnormal physical findings
• Recognizing deviations from mental health baseline, differentiating between dementia and delirium
• Assesses and monitors resident’s actual and potential strengths and limitations including physiological, psychological, socio-cultural, and spiritual needs.
• Taking appropriate action such as consulting with the primary physician or making referral to the Mental Health Team.

Licensed Practical Nurse / LPN Nice to Haves:

• Gerontological experience preferred.
• MDS education (Gold Care or Point Click Care).
• 1-2 years’ experience in a complex care site is preferred .
