Job summary

AgeCare is seeking a Licensed Practical Nurse (LPN) for our South Terrace Edmonton location to deliver goal-directed, patient-centered nursing care in collaboration with an interdisciplinary team.

You will conduct assessments, develop care plans, administer medications, educate residents and families, and coordinate with physicians to ensure high-quality, compassionate care in a supportive, home-like environment. Qualifications

• Current registration as a Licensed Practical Nurse (LPN).
• Experience in senior care or long-term care preferred.
• Education or background in gerontology, rehabilitation, or complex/community care (asset).
• Strong clinical knowledge and care coordination skills.
• Excellent communication, critical thinking, and time management abilities.
• Current CPR and First Aid certification.
• Proficiency with electronic documentation and Microsoft Office.
• Completion of a current Vulnerable Sector Check (or willingness to obtain).

Responsibilities

• Collaborate with the care team using a person-centered approach.
• Conduct and maintain resident assessments, care plans, and documentation; communicate updates as needed.
• Implement and evaluate care plans, adjusting as required.
• Participate in resident care conferences and support scheduling.
• Assist with resident transitions including move-ins, discharges, and transfers.
• Provide direct nursing care and administer medications and treatments in compliance with policies and regulations.
• Safely manage medications and narcotics in accordance with scope of practice and legislation.
• Educate residents and families about health conditions and care plans.
• Coordinate care plans with physicians, team members, and service providers.
• Respond to significant changes in residents’ conditions and manage medical emergencies.
• Provide direction and delegate tasks to unregulated care providers.
• Oversee emergency response in the absence of senior leadership.
• Address resident/family concerns and ensure timely follow-up or escalation.
• Support infection prevention, health and safety initiatives, and quality improvement plans.
