ParaMed in Edmonton is seeking a Home Care Supervisor (LPN) to supervise and support Health Care Aides in community care. You will lead the clinical team, ensure standards of practice, and drive quality patient outcomes.

The role emphasizes leadership, clinical documentation, and collaboration with nursing colleagues to meet funder reporting guidelines while expanding your community nursing expertise.

Qualifications

• Must be a Licensed Practical Nurse with CLHA in good standing.
• 3–5 years’ experience as a practicing nurse.
• Valid driver’s license and reliable transportation.
• Ability to obtain Vulnerable Sector Screening (VSS).
• Strong clinical assessment skills are required.
• Understanding of community nursing practice.

Responsibilities

• Supervise and support HCAs to ensure practices meet standards.
• Respond to concerns from staff, patients, families and team.
• Provide leadership through huddles and one-on-ones.
• Initiate patient safety incident investigations and action plans.
• Collaborate with nursing on documentation for funder reporting guidelines.
• Engage patients with health teaching and problem solving.
• Engage staff with coaching and mentoring to resolve issues.
• Maintain current knowledge of home health care practice changes.
