Job summary

Shannex Incorporated is seeking a Client Medication Assistant to join the Parkland Truro team in Truro, Nova Scotia. The role involves assisting with activities, coordinating pharmacy prescriptions, and providing daily care to residents in a compassionate, professional environment.

Responsibilities include documenting care, communicating health changes, and supporting residents with meals and recreation. A High School diploma and relevant care programs are required; CPR/First Aid is an asset,

Qualifications

  • High School diploma or equivalent education completion.
  • Continuing Care Assistant, Home/Personal Support Worker Program, Special Care Home Worker Program, Health Care Aid or Human Services Program, or equivalent.
  • Parkland Medication Administration Certificate Program or equivalent.
  • Current CPR & Emergency First Aid Certification considered an asset.
  • Basic Food Safety /Food Handlers Course considered an asset.
  • Alzheimer’s Disease and other Dementia Care Course considered an asset.
  • Back in Form Lift and Transfer Program considered an asset.

Responsibilities

  • Assist with planning and implementation of client recreation and leisure activities.
  • Coordinate with CSC for prescription changes and refills with the client’s pharmacy.
  • Plan and organize daily work assignments and prioritize client needs.
  • Communicate health status changes to CSC.
  • Develop awareness of clients’ dietary, allergy, and recreation needs.
  • Monitor at‑risk behaviors and intervene as needed.
  • Escort clients to dining and activities.
  • Complete domestic tasks per service plan (light housekeeping, dining assistance, laundry).
  • Participate in meetings and education programs; document work (Daily Care Flow Sheet, MAR).
  • Maintain continuity of care through communication with health services staff.

About The Opportunity
  • Assisting with planning and implementation of client recreation and leisure activities;
  • Coordinates with CSC for prescription changes and refills with the client’s pharmacy;
  • Participates in and receives report daily. Plans and organizes daily work assignments, and prioritizes the specific needs of the clients;
  • Communicates with the CSC if there is a noted change in the health status of the client e.g. decreases in appetite, increase in confusion, physical weakness, etc.;
  • Develops an awareness of the specific needs of each client e.g. diet, allergies and recreation;
  • Monitors clients assessed for at risk behaviors (exploring, falls, choking, etc.) and intervenes appropriately when necessary;
  • Escorts clients to dining and activities as required;
  • Completes domestic functions for clients according to the service plan including light housekeeping, dining assistance, and laundry;
  • Demonstrates an interest in personal development. Participates on/in: department meetings, committees, internal and external education programs, other programs as required;
  • Completes required documentation of work e.g. Daily Care Flow Sheet, Medication Administration Record (MAR);
  • Communicates with health services staff throughout the shift to ensure continuity of care for clients;
  • Ensures appropriate records are completed and maintained
  • Performs all other duties as assigned.
About You
  • High School diploma or equivalent education completion;
  • Continuing Care Assistant, Home/Personal Support Worker Program, Special Care Home Worker Program, Health Care Aid or Human Services Program, or equivalent;
  • Parkland Medication Administration Certificate Program or equivalent
  • Current CPR & Emergency First Aid Certification considered an asset.
  • Basic Food Safety /Food Handlers Course considered an asset
  • Alzheimer’s Disease and other Dementia Care Course considered an asset.
  • Completion of Back in Form Lift and Transfer Program considered an asset
  • Ability to assess and prioritize multiple work assignments while displaying a high level of professionalism and commitment to quality service
  • Adheres to the Standards of Nursing Practice, Code of Ethics, Registered Nurses Act and Regulations as defined by provincial legislation
  • Maintain current knowledge of organizational policies and procedures including those that govern resident care
  • Current knowledge of literature and applied research for the delivery of health care
  • Current knowledge of community resources (e.g., services and programs), and roles of health care professionals
  • Current knowledge of issues and priorities within health care with a focus on continuing care.
