AgeCare Shawnee in Calgary is seeking an experienced Licensed Practical Nurse (LPN) to join our team. Reporting to the Director of Care, this Regular Part Time role provides planned, patient-centered nursing care in line with provincial standards.

You will work with an inter-professional team, complete care assessments and care/service plans, implement and evaluate them, assist with care conferences, support resident moves, and document care and medication administration in compliance with

Qualifications

• Nurse diploma with current provincial licensure is required.
• 3+ years of experience in seniors' health or geriatric care.
• CPR training and proven computer proficiency including MS Office.

Responsibilities

• Collaborate as part of an inter-professional team following the person-centered care model.
• Complete and maintain resident assessments, care/service plans, and documentation.
• Implement and evaluate care/service plans and adjust as needed for residents.
• Assist with scheduling and participate in resident care conferences as applicable.
• Coordinate with the Director of Care to support moves, transfers and admissions.
• Provide direct resident care and administer medications per regulatory standards.

The Opportunity

AgeCare is seeking an experienced and dynamic Licensed Practical Nurse (LPN) to join our team. Reporting to the Director of Care, this role presents a remarkable opportunity for YOU to make a significant impact on the lives of our residents and their families.

The LPN is responsible for providing planned, patient-oriented, goal-directed nursing care in accordance with the provincial standards of care.

What You Will Do

• Work as a member of the inter-professional team as described by our Professional Practice Model applying the person-centered approach to care.
• Complete and ensure resident's clinical care assessments, care/service plans and documentation are accurate, up to date and changes communicated as required.
• Implement and evaluate the effectiveness of the care/service plans and make changes as required.
• Assist with scheduling of resident's care conferences and participate in conferences where applicable.
• Work closely with the Director of Care to facilitate resident's move in, move out and transfers.
• Provide direct resident care and medication/ treatment administration and document care in compliance with regulatory and Resident Care standards and policies and procedures.

What You Bring

• Practical Nurse Diploma.
• Current registration and membership in good standing in applicable Province.
• 3+ years of experience with seniors’ health.
• Current CPR training.Computer proficiency and experience with Microsoft office applications.
• Demonstrates leadership, coaching and mentorship skills.
