Job summary

AgeCare Shawnee is recruiting a Licensed Practical Nurse (LPN) to join our inter-professional team. Reporting to the Director of Care, you will provide planned, patient-centered nursing care in line with provincial standards, ensuring high quality outcomes for residents and families.

You will perform assessments, develop care/service plans, administer medications, and document care while collaborating with the care team to support moves, transfers and care conferences.

Qualifications

• Practical Nurse Diploma required.
• Current registration and good standing in the applicable province.
• 3+ years experience with seniors' health.
• CPR training current.
• Computer proficiency and Microsoft Office experience.
• Demonstrates leadership, coaching and mentorship skills.
• Values: Trust, Respect, Quality and Teamwork.

Responsibilities

• Work as a member of an inter-professional team following the person-centred care model.
• Complete resident assessments, care plans and documentation; communicate changes.
• Implement and evaluate care plans; adjust as needed.
• Assist with scheduling and participate in resident care conferences.
• Collaborate with the Director of Care on move-in/move-out and transfers.
• Provide direct resident care, medication administration and documentation per standards.

About the Role

The Opportunity AgeCare is seeking an experienced and dynamic Licensed Practical Nurse (LPN) to join our team. Reporting to the Director of Care, this role presents a remarkable opportunity for YOU to make a significant impact on the lives of our residents and their families. The LPN is responsible for providing planned, patient-oriented, goal-directed nursing care in accordance with the provincial standards of care.

What You Will Do

• Work as a member of the inter-professional team as described by our Professional Practice Model applying the person-centered approach to care.
• Complete and ensure resident's clinical care assessments, care/service plans and documentation are accurate, up to date and changes communicated as required.
• Implement and evaluate the effectiveness of the care/service plans and make changes as required.
• Assist with scheduling of resident's care conferences and participate in conferences where applicable.
• Work closely with the Director of Care to facilitate resident's move in, move out and transfers.
• Provide direct resident care and medication/ treatment administration and document care in compliance with regulatory and Resident Care standards and policies and procedures.

What You Bring

• Practical Nurse Diploma.
• Current registration and membership in good standing in applicable Province.
• 3+ years of experience with seniors' health.
• Current CPR training.
• Computer proficiency and experience with Microsoft office applications.
• Demonstrates leadership, coaching and mentorship skills.
