Job summary

Health PEI is seeking a Licensed Practical Nurse for the Transition Unit in Charlottetown. The position is part-time (80%) and permanent, commencing immediately, working within a provincial addiction treatment facility.

Responsibilities include patient assessment, care, medication administration, documentation, and coordination with the care team to support withdrawal and recovery processes. Applicants must be eligible for PEI nursing registration and maintain current certifications.

Qualifications

• Successful completion of an approved LPN program.
• Active or eligible for registration with the Prince Edward Island College of Nursing & Midwifery.
• Demonstrates ability to communicate effectively in writing and orally.
• Capable of sustained physical activity and maintaining good attendance.

Responsibilities

• Assist with and complete assessment on new admissions and document medical history.
• Provide personal care to assigned patients during each shift.
• Administer medications and observe patient responses to treatments.
• Monitor patients to maintain safety and prevent unauthorized absences.
• Support creation and implementation of the case plan during withdrawal process.
• Document client notes daily and communicate progress at shift changes.
• Transport patients to sessions and appointments with the in-house doctor.
• Perform other duties as required.

The Licensed Practical Nurse will work under the direction of the Registered Nurse in caring for patients admitted to the Transition Unit.

Duties

• assisting with and completing assessment and gathering information on new admissions including documenting a recent medical, psychiatric and substance abuse history assessing patients in withdrawal, observing for signs and symptoms of withdrawal seizures, DTs and hallucinations;
• assisting with screening for admission, admissions and discharges;
• providing personal cares to an assigned patient list during each shift;
• administrating medication, and observing and reporting patient reaction and response to treatments and medication;
• monitoring and observing patients, making it safer for other patients and preventing unexpected absence without leave attempts;
• helping maintain a therapeutic milieu for growth of patients and implementing behavioural management techniques;
• assisting in the implementation of the case plan through encouraging and supporting clients in the withdrawal process;
• daily documentation of client notes and keeping other team members informed of progress or issues at shift change;
• transporting patients to various sessions and taking patients to appointments with the in-house doctor;
• performing other duties as required.

Minimum Qualifications

• successful completion of an approved course of study for Licensed Practical Nurses.
• active or eligible for registration with the Prince Edward Island College of Nursing & Midwifery.
• demonstrates application of knowledge of client’s physical, psychosocial, emotional and spiritual needs, which includes clients with chronic illnesses.
• demonstrates an alignment with health PEI’s mission, vision and values, including honesty, integrity in personal and professional behaviour.
• capable of sustained physical activity.
• have a good previous work and attendance record.
• current in CPR, BCLS, TLR, WHIMIS All Hazards Codes, and annual re-certifications, required;
• demonstrated a professional presence and model professional behaviour;
• knowledge of chronic illnesses is required;
• must be able to work well independently and under supervision.
• ability to work as a member of the clients care team (interdisciplinary team), requesting and accepting direction as required;
• ability to communicate effectively, both orally and in writing.
• demonstrated effective time management and organizational skills.
• able to provide physical, psychosocial, emotional and spiritual care to the client being cared for.
• The successful applicant must provide a satisfactory criminal record check and vulnerable sector checks prior to beginning employment

Other Qualifications

• Previous experience working with clients with substance use would be considered an asset.
• Additional relevant education and experience will be considered an asset.

Salary Range

$32.42 - $38.67 per hour (Level LPN - UPSE)

Bi-Weekly Hours

60 Hours Bi-Weekly / Rotating / 8-Hour & 12-Hour Shifts / Weekdays / Evenings / Nights / Weekends
