We are looking for a Licensed Practical Nurse to serve as a Supervisor, providing high-standard care at Seasons. This role involves maintaining a safe, secure environment for residents, families, and staff, while mentoring and guiding our care team.

What You Will Do:

  1. Utilize the nursing process to deliver high-quality care to residents.
  2. Perform supervisory duties, ensuring excellent care and providing ongoing mentorship and education to the care team.
  3. Assess residents transitioning into Seasons and follow up at 30 days, annually, or with any significant change, in accordance with CCHSS guidelines.
  4. Prioritize resident needs and establish care plans accordingly.
  5. Ensure compliance with Resident Assessments, Service Plans (PCC), and community policies, procedures, and AHS regulations.
  6. Manage risks through thorough monthly care audits.
  7. Administer medications; PCC knowledge is an asset.

Qualifications and Skills:

  • Current Licensed Practical Nurse license in good standing with CLPNA.
  • Current First Aid and CPR certification.
  • Excellent verbal and written communication skills.
  • Preparedness to respond to emergencies.
  • Experience with seniors in retirement or long-term care settings is an asset.
  • Strong leadership and supervisory abilities with residents and staff.
  • PCC knowledge is an asset.
