About The Role
The Licensed Practical Nurse (LPN) reports to the Director of Care and/or designate. This position is a role model and team leader within an assigned community, responsible for coordinating care delivery for the persons served. The LPN performs duties and monitors departmental activities in accordance with their scope of practice, established standards, regulations, policies, and procedures to maximize individualized and resident-centered care.
Responsibilities
- Be an engaged collaborative team player, developing and maintaining professional relationships that support and strengthen the organizational culture and purpose.
- Assess residents’ needs; implement and evaluate individualized resident-centered care plans.
- Ensure residents' rights are observed and adhere to a holistic philosophy of wellness.
- Liaise with physicians, community agency representatives, families, visitors, volunteers, other disciplines, and departments to ensure quality resident-centered care.
- Assess and treat all wounds on the unit and act as a resource to care staff for strategies to prevent skin breakdown. Consult with RN or designate for changes in treatment strategies as needed.
- Administer prescribed medications and treatments within the limitations of the BC College of Nurses and Midwives (BCCNM).
- Monitor and report changes in physical and psychological conditions of residents. Document changes on the resident health record, and notify family and physician of changes in condition.
- Participate actively in Interdisciplinary Care Conferences.
- Participate in care plan reviews to ensure current and individualized care plans.
- Support administrative requirements at the community in accordance with established standards, including admissions, discharges, and maintaining confidentiality.
- Participate in orientation of new staff, community programs, meetings, committees, and educational in-services.
- Provide coaching and direction to care staff as needed, using in-the-moment education to improve resident care, including completing all Point of Care (POC) documentation. Seek assistance/support from RN or designate as needed.
- Maintain up-to-date knowledge of community policies and procedures, especially those concerning emergency procedures and equipment management.
- Perform other related duties as required.
Qualifications and Skills
- Graduate of an approved Practical Nursing program and hold a current license with the British Columbia College of Nurses and Midwives (BCCNM) as an LPN.
- Valid Occupational First Aid Level 1 certification.
- Successful completion of annual Resident Assessment Instrument (RAI) competency testing.
- Previous experience in a long-term care facility is an asset.
- Must pass the vulnerable sector Criminal Record Check as per Provincial guidelines (see BCCNM license).
- Proficient in reading, writing, and speaking English, with the ability to understand verbal and written instructions.
- Computer skills including navigating basic programs such as Word, Excel, and Outlook.
- Physical ability to push, pull, stoop, crouch, lift, and carry up to 25 lbs. Must be able to work with cleaning agents and equipment.
