Job summary

Alberta Health Services is seeking a Licensed Practical Nurse for designated float Home Care in rural communities around Okotoks. You will work with an inter-professional team, assess and deliver direct nursing care across the life span, and coordinate with the RN Case Home Care Manager to adjust plans as needed.

The role requires travel in a rural area, a valid driver’s license, and the ability to travel 40 km south of Calgary. Prior home care experience and BCLS-HCP are essential.

  • Completion of an accredited practical nurse education program.
  • Active or eligible for registration and a practice permit as a Licensed Practical Nurse with CLPNA.
  • Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP).

This exciting opportunity as a Licensed Practical Nurse (LPN) - Designated Float covers planned and unplanned staff absences.If you enjoy a role in providing services to clients in a community-based settings in rural communities, then this position will be of interest to you.The Licensed Practical Nurse (LPN) in Home Care works as a member of an inter-professional health care team utilizing the nursing process to provide direct nursing care to individuals across the life span.The LPN is responsible for her/his nursing practice as defined by the College of Licensed Practical Nurses of Alberta (CLPNA).In collaboration with the Registered Nurse Case Home Care Manager, the LPN is responsible to assess, plan, implement and evaluate the needs of Home Care clients and their caregivers/families.Based on the Continuing Care Case Management Competency Framework, you demonstrate ability in the areas of personal and professional accountability, systems thinking, facilitating creative thinking, communicating and building relationships.Using a person-centered approach, you will develop a care plan supporting the client/family to identify needs, focus on strengths, set achievable personal health and self-care goals and provide direct nursing care to individuals across the life span.In collaboration with the client/family, you will ensure services are provided and modified on an ongoing basis, based on client’s response to interventions, incorporating stewardship into service authorization.This role requires flexibility, adaptability and rural/remote travelling; possession of a valid drivers’ license and access to a motor vehicle in good repair for work related purposes.This role requires travel 40 km south outside of the Calgary City Limits. This position supports teams Diamond Valley, Okotoks, and High River/Nanton.

Required Qualifications:

Additional Required Qualifications:

Completion of a post-basic Medication Administration Course for LPNs.You will be responsible to carry and move equipment, use and operate a laptop computer and mobile phone, and for manual handling (lifting, lowering, carrying, pushing and pulling upwards to 50 Ibs) as a regular part of the job.Learning resources and training are provided to safely perform work.The candidate must have at least 5 years of nursing experience with a minimum of 3 years of recent Home Care nursing experience.Level 2 McMaster AHS Case Management education.

Preferred Qualifications:

Experience working with acute clients and/or long-term clients with a focus on chronic disease management. Experience providing case management, client teaching and professional nursing interventions including but not limited to Home Parenteral Therapy (HPT), wound care, ostomy care.Experience supporting vulnerable clients, including those requiring palliative, mental health and/or addictions support. Recent work experience in a community-based setting.Home Care experience preferred.
