Job summary

SE Health, via the Elizz team, is hiring casual Licensed Practical Nurses in Vancouver to provide holistic home-based nursing care. You will work within the LPN scope, collaborating with families and health professionals to meet physical, emotional and spiritual needs.

Qualifications

• LPN Certificate with active BCCNM registration.
• Current First Aid (Standard) / CPR Certification.
• Valid driver’s license and access to a vehicle.
• TB test within 12 months and immunization records.
• Vulnerable sector check completed or in process at hire.
• Availability to work days, evenings and weekends.
• Able to work independently and as part of a team; strong organization.
• Basic computer/smartphone skills; comfortable with tech.

Responsibilities

• Provide hands-on care to residents per Care Plan and policies.
• Report changes in resident conditions promptly to decision makers.
• Supervise and support non-regulated care partners delivering the plan.
• Identify health, safety or maintenance issues and escalate.
• Liaise with AHS, families and health professionals to ensure high-quality care.
• Participate in team meetings and ongoing training.
• Chart daily using the electronic charting system.
• Perform duties safely and efficiently.

Responsibilities:

• Provide hands-on care to residents according to the Care Plan and Community policies and procedures.
• Report changes in resident conditions as well as any other pertinent information to the appropriate decision makers in a timely manner.
• Assist in the monitoring of non-regulated care partners assigned to carry out the care plan, including supervising, teaching delegated tasks, coaching, and supporting as per the hospitality model.
• Identifies health, safety or maintenance issues, and reports as required.
• Liaising with AHS and other health care professionals, families, and any other people required in order to facilitate the highest level of resident care.
• Actively participates in team meetings and training/learning opportunities as required.
• Charting on daily basis using electronic charting system
• Perform all duties in a safe and efficient manner.

Requirements:

• Hours are 2 hours with a client once a month
• LPN Certificate, current active registration with the BCCNM in good standing.
• Current First Aid (Standard) / CPR Certificate (Level C)
• A valid driver’s license and vehicle.
• Current immunization record including (TB test completed within the past 12 months) is required upon hire
• Current vulnerable sector check screening is required upon hire
• Availability to work days ORevenings; includingweekends
• Able to work independently as well as part of a team, with excellent organizational skills to manage your schedule
• Well-developed interpersonal and customer service skills, to effectively communicate with clients and to provide companionship to those who need it
• Effective problem-solving and conflict management skills
• Basic computer/smartphone skills would be an asset.
• This is a physically demanding position and requires hand, arm and shoulder dexterity, sensory ability, lifting to a maximum of approximately fifty-five (55) pounds, walking, climbing stairs, standing, bending, reaching, and frequent movement
