The Role

The Licensed Practical Nurse (LPN) collaborates with a multidisciplinary team to deliver care tailored to residents with well-defined and stable health conditions. The LPN assesses, plans, implements, and evaluates care, consulting with colleagues to meet more complex needs.

Responsibilities

• Monitor and establish standards of care
• Observe and report changes in residents’ health
• Process physician’s orders
• Develop and maintain resident Care Plans using MDS-RAI
• Supervise nursing care and HCAs
• Administer medications and evaluate their effectiveness
• Collaborate with pharmacists
• Document and communicate resident status changes

Qualifications

• CLPNA license
• CPR certification
• Experience in geriatric, rehab, or long-term care nursing (preferred)
