Job summary

Monarch Health Solutions Inc. is seeking licensed practical nurses (LPNs) to provide compassionate in-home nursing care for seniors in Victoria, BC.

Both part-time and full-time opportunities are available, with a focus on promoting independence and comfort in patients’ homes. Responsibilities include wellness visits lasting 30–40 minutes, care plan adherence, patient education, and collaboration with physicians and the interdisciplinary team.

Qualifications

• Current LPN license and CPR-C certification.
• Reliable vehicle and a valid driver’s license for travel to patient locations.
• Ability to perform comprehensive nursing assessments and document findings clearly.

Responsibilities

• Conduct wellness home visits with 30–40 minute appointments for chronic disease care.
• Deliver nursing care per individualized care plans, regulatory guidelines, and patient safety standards.
• Monitor patient status, identify concerns early, and support preventative and treatment-based care.
• Perform comprehensive nursing assessments including cardiovascular, respiratory, skin, and general health checks.
• Document findings, interventions and changes in condition accurately.

We are seeking skilled and compassionate Licensed Practical Nurses (LPNs) to join our team. Key Responsibilities

• Conduct wellness nurse home visits, providing 30–40 minute appointments to support stable patients managing chronic diseases and maintaining their independence.
• Deliver exceptional nursing care in accordance with individualized care plans, best practices, regulatory guidelines, and patient safety standards.
• Act as the eyes and ears of the physician/GP by monitoring patient status, identifying concerns early, and supporting preventative and treatment-based care.
• Perform comprehensive nursing assessments, including cognitive, cardiovascular, respiratory, lower limb, skin integrity, and general health assessments.
• Monitor patient progress and accurately document findings, interventions, and changes in condition.
• Address Activities of Daily Living (ADLs), including social well-being, mental health, sleep, mobility, personal care needs, and overall wellness.
• Provide patient and family education to promote wellness, recovery, independence, proper foot hygiene, footwear, and self-care strategies.
• Coordinate care on behalf of the GP, including arranging diagnostics, labs, referrals, and communication with interdisciplinary team members.
• Provide nursing services such as wound care, vaccinations, personal care, foot care, and charting by exception.

Skills and Qualifications:

• Positive, can-do attitude with excellent relational and observational skills.
• Proficiency in checking vital signs and conducting comprehensive assessments.
• Strong time management and attention to detail.
• Pharmacological knowledge.
• Current nursing license (LPN) and CPR-C certification.
• Reliable vehicle and a valid driver’s license for travel to patient locations.

Compensation:

• Part-Time $45/hour
