Job Description

The LPN ensures the well-being of residents by delivering high-quality services consistent with Chartwell Retirement Residences' policies, procedures, and philosophy.

Responsibilities include but are not limited to:

• Providing assessments, treatment, personal care, medication administration, and delegated health services to residents, as well as supporting and educating Health Care Aides;
• Communicating with residents and families to meet needs and ensure satisfaction;
• Understanding residents' heritage, values, aging process, and disabilities;
• Facilitating family involvement;
• Administering medications within scope and regulations;
• Performing delegated nursing skills accurately;
• Completing assessments and observing resident health, reporting to the Assisted Living Manager, and acting as appropriate;
• Providing services as per the Care Plan and standards;
• Documenting services and charting as per policies;
• Monitoring and recording residents' physical and mental conditions;
• Documenting incidents in the resident record.

Ideal Candidates Will Possess:

• Knowledge of geriatric, rehabilitation, or long-term/retirement nursing, including relevant legislation;
• Graduation from a recognized LPN program and valid registration with LPN associations;
• Empathy, understanding of seniors' needs, strong customer service, and conflict resolution skills.
