Job Description

The LPN ensures the general well-being of residents through the delivery of high-quality services in a manner consistent with the policies, procedures, and philosophy of Chartwell Retirement Residences.

The responsibilities of this position include, but are not limited to:

  1. Providing basic assessments and treatments, personal care, medication administration, and delegated health services to residents, as well as supporting, guiding, and educating Health Care Aides;
  2. Communicating with residents and families as required to ensure needs are met and services are satisfactory;
  3. Demonstrating appreciation of residents' heritage, values, and wisdom, and understanding the aging process and living with a disability;
  4. Facilitating and encouraging family involvement with residents;
  5. Administering medications within the LPN scope of practice, in accordance with regulations and standards;
  6. Performing delegated nursing skills accurately and skillfully;
  7. Completing routine assessments and continuous observation of residents' health status, reporting observations to the Assisted Living Manager, and acting upon observations as appropriate;
  8. Ensuring services in the Care Plan are provided according to established standards and at approved levels;
  9. Proper documentation of services and charting as per policies and procedures;
  10. Monitoring and maintaining records of residents' physical and mental conditions;
  11. Documenting resident incidents in the client record.

The ideal candidates will possess:

  1. A sound knowledge and background in geriatric, rehabilitation, or long-term/retirement nursing, including activation, nursing, and personnel administration. Knowledge of relevant federal, provincial, and municipal statutes such as the Health Disciplines Act, Occupational Health and Safety Act, Pharmacy Act, Food and Drugs Act (Canada), and Narcotic Control Act.
  2. Graduation from a recognized LPN program and valid registration with appropriate LPN associations.
  3. The ability to demonstrate empathy, understanding of seniors' needs, strong customer service orientation, and conflict resolution skills.
