Job summary

ParaMed is Canada’s leading provider of home health care services. As an LPN, you will conduct scheduled community visits to care for patients with support from RNs for more complex cases, serving individuals of varying ages in homes to promote comfort and independence.

This role offers autonomy, a balanced schedule with every other weekend off, and opportunities to develop long-term therapeutic relationships while practicing and expanding complex clinical skills.

Qualifications

  • Active LPN registration with CLPNA.
  • Valid driver’s license and access to a vehicle.
  • Experience in delivering community-based care is an asset.
  • Reliability and flexibility for varied schedules.
  • Willingness to travel to patient residences within a designated area.
  • Experience with vulnerable populations and patient safety focus.
  • On-call coverage, including after hours, is required.

Responsibilities

  • Participate in scheduled community visits to assess patient needs within scope of practice.
  • Provide ethical and safe nursing care and monitor care plans.
  • Use assessment tools to identify patient needs and adjust plans accordingly.
  • Build rapport with patients and maintain clear, updated records.

In this community nursing role, you as a Licensed Practical Nurse (LPN) will have scheduled visits within the community to care for patients in various settings with the support of RNs for more complex patients. You will work with patients of all ages with differing complexity of care needs and play a key role in helping patients remain comfortable in their homes for as long as possible. What you’ll do:
  • Participate in scheduled visits in the community to look after patient needs within your scope of practice
  • Provide ethical and safe nursing care
  • Use appropriate assessment tools/techniques to assess patients’ individual needs
  • Execute and continuously monitor care plans and adjust as required
  • Engage patients using a collaborative approach by building rapport and trust
  • Maintain and update professional patient records related to patient encounters and plans
What you bring:
  • Registration with the CLPNA as a LicensedPractical Nurse with 'Active' status
  • Valid driver’s license and access to a vehicle is required
