Job summary

ParaMed Home Health Care is seeking a Registered Practical Nurse to perform scheduled community visits and deliver compassionate nursing care within your scope of practice. You will work with patients of varied ages, with different care needs, supported by RNs for complex cases, promoting independence and comfort at home.

You bring CNO-registered status, a valid driver’s license, and a willingness to travel across a designated area.

Qualifications

• RPN with valid CNO registration in good standing.
• Valid driver’s license and access to a vehicle.
• Willingness to travel to patient residences within a designated area.
• Experience in delivering community care is an asset.
• On-call coverage, including after hours, to support patient safety.

Responsibilities

• Participate in scheduled community visits to address patient needs within your scope of practice.
• Provide ethical and safe nursing care.
• Use assessment tools to identify patient needs and adjust plans accordingly.
• Execute and monitor care plans, updating as required.
• Engage patients with a collaborative approach to build rapport and trust.
• Maintain accurate patient records and care plans.

In this community nursing role, you as a Registered Practical Nurse (RPN) will have scheduled visits within the community to care for patients in various settings with the support of RNs for more complex patients. You will work with patients of all ages with differing complexity of care needs and play a key role in helping patients remain comfortable in their homes for as long as possible. What You’ll Do

• Participate in scheduled visits in the community to look after patient needs within your scope of practice
• Provide ethical and safe nursing care
• Use appropriate assessment tools/techniques to assess patients’ individual needs
• Execute and continuously monitor care plans and adjust as required
• Engage patients using a collaborative approach by building rapport and trust
• Maintain and update professional patient records related to patient encounters and plans

• Registration with the CNO as a Registered Practical Nurse in good standing
• Valid driver’s license and access to a vehicle is required
