Qualifications

  • Registration as a Licensed Practical Nurse with CLHA in good standing.
  • Minimum of 3–5 years’ nursing experience.
  • Valid driver’s license and access to reliable transportation.
  • Able to provide a current, clear Vulnerable Sector Screening (VSS).
  • Comprehensive patient health assessment skills.
  • Understanding of community nursing practice.
  • Proven experience leading and managing a team.
  • Knowledge of clinical documentation standards and practice guidelines.
  • Excellent organizational, interpersonal, and teaching skills.
  • Commitment to professional development in home health care.
  • Effective critical thinking and problem-solving skills.

Responsibilities

  • Supervise and support HCAs to ensure practices comply with legislated standards and company policies.
  • Respond to and advise/resolve concerns from community staff, patients/families, and team members.
  • Provide ongoing leadership through regular huddles, meetings and one-on-ones.
  • Initiate patient safety incident investigations and ensure action plans are followed.
  • Collaborate with nursing team to support clinical documentation and funding guidelines.
  • Engage patients in problem solving, health teaching, and building rapport to identify needs and goals.
  • Engage staff in problem solving with education, coaching, and mentoring.
  • Maintain up-to-date knowledge and skills to provide safe, efficient care across practice areas.

In this valuable role, the Clinical Supervisor is accountable for the supervision of Health Care Aides- who are caring and making a difference for clients within the community. The Clinical Supervisor will have the expertise to assess, implement and evaluate a wide range of clinical care interventions to ensure the delivery of safe, quality, and ethical care.

What you’ll do:
  • Supervise and support HCAs to ensure practices comply with all legislated standards of practice as well as company policies and procedures.

  • Respond to and advise/resolve concerns from community staff, patients/families, and other team members.

  • Provide ongoing leadership through regular huddles, face to face meetings and one-on-ones.

  • Initiate patient safety incident investigations and documents to ensure timelines and action plans are met.

  • Work collaboratively with nursing team members to support clinical documentation and submission requirements according to specific funder reporting guidelines.

  • Engage patients in problem solving using a collaborative approach, offering health teaching, building rapport and trust to identify patient health care needs, strengths, capacities, and goals.

  • Engage staff in problem solving, offering education, coaching, and mentoring in resolution of problems.

  • Maintain the knowledge, skills, and attitudes to provide safe, efficient, effective service in all areas of practice by keeping abreast of changes in practice setting that affect scope of practice.

What you bring:
  • Registration as a LicensedPractical Nurse with CLHA in good standing
