Reporting to the Resident Care Manager, the Licensed Practical Nurse (LPN) is responsible for managing the day-to-day delivery of care to residents by motivating and empowering the team. The LPN works cohesively with members of the interdisciplinary team to ensure the nursing process (Assessment, planning, implementation and evaluation) is individualized and ongoing, such that changing needs of residents are addressed and met.
In accordance with Covenant Care’s mission, vision, and values, this position is responsible for ensuring that residents receive quality care and service according to Covenant Care policies and procedures. Provides support to residents, their family and visitors during the resident’s end of life journey that will enable choice, dignity, privacy, respect and individuality.
