Responsibilities

• Conduire des initiatives régionales de planification et d’intégration du système de santé.
• Utiliser les données de population, financières, qualité et performances pour identifier les écarts et recommander des solutions.
• Établir des partenariats avec les prestataires, le gouvernement et les Ontario Health Teams pour une prise en charge intégrée.
• Conduire des initiatives avec communication claire et résolution collaborative de problèmes avec les dirigeants seniors.
• Élaborer des plans stratégiques et opérationnels alignés sur la direction provinciale, adaptés au nord-ouest.
• Suivre le financement, les contrats et les indicateurs de performance pour assurer l’amélioration continue.

You will lead stakeholder engagement, use data to identify opportunities, and design and implement clinical standards, programs, and pathways that improve capacity, performance, and health outcomes.
