Job summary

ByWard Family Health Team is seeking an experienced Nurse Practitioner to join our interdisciplinary primary care team in a permanent, full-time role. You will take ownership of an established patient panel of approximately 1,700 patients with access to a broad interdisciplinary team for support.

You will independently assess, diagnose, treat and manage a wide range of health conditions, order investigations, prescribe medications, provide preventive care, and coordinate referrals within a fully

Qualifications

• Current registration as a Nurse Practitioner in Ontario.
• Graduate of a Nurse Practitioner program.
• Minimum two years NP experience, preferably in primary care.
• Experience with EMR and patient panel management.
• Bilingual French/English is an asset.

Responsibilities

• Provide longitudinal primary care to your patient panel.
• Assess, diagnose and manage acute and chronic conditions.
• Order tests, prescribe medications and create treatment plans.
• Provide prevention, screening and health education.
• Coordinate referrals and follow-ups with specialists.
• Collaborate with the care team to ensure coordinated care.
• Maintain thorough documentation and support quality initiatives.
