Job summary

Flemingdon Health Centre in Toronto is hiring a Nurse Practitioner to be deployed with the Canadian Family Clinic care team. This 0.5–1.0 FTE role offers a varied schedule including evenings and weekends as part of a leading primary care initiative serving Flemingdon Park, Fairview and Thorncliffe Park.

The NP will work to full scope under CNO standards, provide assessment, diagnosis, treatment, health promotion, education and referrals, and collaborate with interdisciplinary colleagues to

Qualifications

• Registered Nurse in the Extended Class with the College of Nurses of Ontario.
• Baccalaureate in Nursing and completion of a recognized Primary Care Nurse Practitioner program.
• Two to three years of primary care or community nursing experience is an asset.
• Experience with chronic disease management and health equity.
• Willingness to work evenings and weekends; flexible hours.
• Strong communication and teamwork skills.

Responsibilities

• Provide comprehensive primary care including assessment, diagnosis and treatment within NP scope.
• Order and interpret screening tests; prescribe designated drugs as authorized by the CNO.
• Maintain accurate records and coordinate care with health teams and community partners.
• Support health promotion, disease prevention and quality improvement initiatives.
