Job summary

Intercare Brentwood Care Centre in Calgary, AB, is seeking a dedicated Case Manager RN to lead the care team at the unit level. The role focuses on delivering high-quality resident care, implementing IP&C guidelines, and supporting the care team through planning, coordination and evaluation of services.

You will drive continuous quality improvement, coordinate admissions and care conferences, and work with HR on recruitment and staff development.

Qualifications

• Graduate of an accredited School of Nursing with current registration as a Registered Nurse (CRNA). Bachelor’s Degree in Nursing preferred; or an LPN with active registration.
• A minimum of three (3) years of related care experience, preferably in continuing care.
• A minimum of two (2) years of leadership experience.
• Demonstrated flexibility, organizational and time management skills.
• Excellent communication and presentation skills with attention to detail.

Responsibilities

• Promote Intercare’s vision by upholding Core Values and ensuring dignity, choice and growth in care services.
• Plan, organize, and evaluate Resident Care Services in alignment with standards and regulations.
• Participate in resident admissions and care conferences.
• Lead infection prevention and control activities and conduct audits of practice.
• Manage outbreaks per Alberta Health Services guidelines at the unit level.
• Monitor care delivery and mentor front-line staff to maintain quality of care.
• Facilitate care planning and interdisciplinary team assessments for complex residents.
• Act as a resident advocate and support resident/family involvement.
• Lead recruitment, workforce planning, and performance management in collaboration with HR.
• Ensure resource management for budget and supply utilization.
