Job summary

Nova Scotia Health is seeking a Health Care Social Worker to join an interdisciplinary health care team. You will provide holistic care across the care continuum, addressing psychosocial, emotional and spiritual wellbeing for patients and families.

The position requires an MSW, current Nova Scotia College of Social Workers registration, and at least 2 years of post-MS W clinical experience. Travel to other sites may be required; French is a plus.

Qualifications

  • Master's degree in Social Work from a recognized university.
  • Current registration with the Nova Scotia College of Social Workers.
  • 2 years' post MSW related clinical experience required.
  • Previous palliative care experience an asset.
  • Grief & Bereavement Training an asset.
  • Previous experience working in community-based setting preferred.
  • Demonstrated ability to use a client-/family-centered approach.
  • Ability to complete biopsychosocial assessments.
  • Demonstrated ability to assist patients and families with navigation of local community resources and agencies.
  • Travel to other sites may be required and must have access to a reliable vehicle.
  • Competencies in other languages an asset, French preferred.

Responsibilities

  • Provide collaborative, holistic care on the primary to quaternary care continuum.
  • Engage clients and families to assess, plan, intervene and evaluate psychosocial needs.

About the Opportunity

The Health Care Social Worker is a member of the interdisciplinary health care team providing collaborative, holistic care on the primary to quaternary care continuum. Social Workers are primarily concerned with the relationships between people and their environments, which are integral to psychosocial, emotional and spiritual wellbeing. The social determinants of health are embedded in social work theory and practice and are the focus for the dynamic and interactive process of engagement, assessment, planning, intervention and evaluation. Social Workers build upon the strengths of patients/clients by focusing on existing coping strategies and problem-solving skills, past successes, and personal resources.

About You

Hours
  • Permanent full-time position; 75 hours bi-weekly
Compensation and Benefits

$40.17 - $55.49 Hourly
Successful candidates may be eligible for our benefits package which includes health, dental, travel, long-term disability, and life insurance coverage as well as a defined benefit pension plan.
