Physical Therapist

Purpose of this position

Evaluate, treat, document, and facilitate care for residents with impaired ability to function due to disease. Participate in interdisciplinary team meetings and assist in developing decline prevention policies.

Essential Job Duties & Responsibilities
  1. Evaluate and document resident assessments in line with facility policies.
  2. Provide treatments to help residents maintain or improve their function.
  3. Recommend appropriate treatments, preventive measures, and adaptive equipment.
  4. Maintain accurate reports as required by regulatory agencies.
  5. Follow infection control procedures.
  6. Ensure resident safety under supervision.
  7. Complete Minimum Data Set assessments as assigned.
  8. Document evaluations, treatment plans, and progress notes timely.
  9. Participate in care conferences and quality improvement programs.
  10. Collaborate with residents, families, staff, and healthcare providers.
  11. Follow all facility policies, safety guidelines, and residents' rights policies.
  12. Adhere to compliance and confidentiality standards.
  13. Present a professional appearance and dependable attendance.
  14. Perform other duties as assigned efficiently and professionally.
Qualifications
  • Excellent verbal and written communication skills.
  • Ability to analyze medical information and communicate effectively with residents and professionals.
  • Problem-solving skills and familiarity with Word and Excel.
  • Doctorate of Physical Education.
  • Active Physical Therapist Licensure.
