Nova Scotia Health Authority is seeking an Interdisciplinary Clinical Leader for Rehabilitation Services at Soldiers Memorial Hospital. Reporting to the Manager, the Interdisciplinary Clinical Leader provides leadership, innovation and support to their clinical program areas, with the goal of providing collaborative person-centered care to patients and families. The inter-professional clinical leader works closely with the team/program managers, professional practice coordinators, physicians, nurse leaders and other team members, to promote person-centered, collaborative care by employees within the Rehabilitation Services portfolio. Roles and responsibilities may include: fostering healthy inter-professional team processes that support person centered care across the continuum, identifying staffing needs and ensuring staffing requirements are being met on a daily basis, assisting in the development of client centered staffing models, fostering team processes that encompass the continuum of patient care from admission through discharge into the community, program planning and evaluation - ensuring practice is inter-professional and evidence informed, participating in the recruitment, selection and orientation of new team members, and supporting a quality approach to service delivery.
