Job summary

Wesley Halton Housing is hiring an Intensive Case Manager for evenings and weekends to support clients in a housing-first approach. You will handle intake, coordinate with landlords, and assist families toward stable housing in Halton Region.

Ideal candidates hold a Human Services degree, are registered with the OCWSSW, and have experience with mental health, addictions, and homelessness. Flexibility for shifts and travel is required.

Qualifications

  • Post‑secondary degree/diploma in Human Services or related field.
  • Registration and member in good standing with the Ontario College of Social Workers and Social Service Workers (OCSWSSW) is required.
  • Experience working with adults and families with mental health and addictions issues and the homeless and providing non-judgmental supports to them.
  • Must be able to work flexible shifts including day, evening and weekend hours as well as 24‑hour intake responsibilities

Responsibilities

  • Works with assigned families and individuals in a housing program which works towards a housing plan.
  • Responds to calls and referrals from community agencies in accordance with case load
  • Places individuals and families into permanent leased units, securing appropriate housing based on individual need.
  • Assesses and screens participant’s/families’ needs for services to support their transition to stable housing and optimize their capacity to sustain housing using an acuity tool and other resources.
  • Completes Social & Community Housing applications (HATCH) to submit to the Region, including all required documents for approval OR ensures existing application remains active.
  • Assess clients for any recent, current or ongoing experience with domestic violence and make referrals to appropriate programs.
  • Assist participants with their move into independent accommodations and provide neighborhood orientation.
  • Maintain regular, effective communication with social housing providers, administrators, community partners and private landlords, to ensure an integrated approach that will provide the best support possible for clients to maintain housing.
  • Report to appropriate child welfare agency (CAS) if child abuse and/or neglect is suspected or witnessed.
  • Conducts regular visits with participants in their homes or shelter to provide supports specific to life skills, daily living skills, conflict resolution, crisis intervention and prevention, budgeting assistance, financial literacy, community integration and community building, grocery shopping, cleaning, etc.
  • Maintain appropriate and accurate record keeping practice, as per expectations of Wesley.
  • Supports approximately 1 shift per month of 24-hour intake; flex time is available for additional hours worked.
  • Other duties as assigned.

Are you the one we’re looking for?

We are seeking a dynamic, highly motivated Intensive Case Manager who demonstrates our values of compassion, accountability, responsiveness and empowerment; to our clients, families and each other.

Reporting to the Manager of Halton Housing Services, this position will assist in providing intake supports and carrying out day‑to‑day administrative services. Wesley Halton Housing offers services to the homeless in Halton Region, supporting homeless families and individuals in a transitional program on‑site. Services are based on a client‑centered, residential harm reduction, housing first approach to service delivery.

This is a unionized, full‑time permanent position. The rate pay is $25.11 per hour, based on 40 hours per week. This position requires availability for evening and weekend hours.

Key Responsibilities:
  1. Works with assigned families and individuals in a housing program which works towards a housing plan. This includes, maximizing income, filing taxes, obtaining ID and connecting with community supports.
  2. Responds to calls and referrals from community agencies in accordance with case load
  3. Places individuals and families into permanent leased units, securing appropriate housing based on individual need.
