Job summary

Oak Valley Health in Ontario is seeking a Registered Practical Nurse to join the Medicine and Transitional Bed Unit. You will perform comprehensive assessment, care planning, patient education and discharge planning to support continuity of care during the inpatient stay.

The role emphasizes activation, professional practice, and patient-centered care within an interprofessional team. Schedule features 12-hour day and night shifts (0730-1930 and 1930-0730) on weekdays, weekends and holidays,

Qualifications

• Graduate from a recognized Registered Practical Nursing diploma program.
• Current RPN registration with the College of Nurses of Ontario.
• Current BLS certification.
• Recent medical/surgical experience required.

Responsibilities

• Conduct comprehensive patient assessments and develop care plans.
• Provide patient education and discharge planning to support continuity of care.
