Job summary

Cencora is seeking a skilled nurse to support patient and provider services in Canada through Innomar Strategies. The role focuses on safe care coordination, documentation, and multidisciplinary teamwork across clinics and homecare settings.

The successful candidate will have a nursing degree, active licensure, and at least 2 years of practical nursing experience, with travel within the designated region expected.

Qualifications

• Bachelor's degree in nursing or related field is required.
• Active nursing license/registration in the applicable jurisdiction is required.
• Minimum 2 years' experience as a Registered Practical Nurse (RPN) or Licensed Practical Nurse (LPN).

Responsibilities

• Administer medications (IM, SC, IV) in home or clinic settings.
• Support/monitor IV infusion treatments in clinic/homecare.
• Educate patients per program protocols, individually or in groups.
• Document and report adverse events per guidelines.
• Coordinate with patients, case managers, physicians and teams to schedule treatments.
