Job summary

SOAHAC in Cambridge, ON is seeking an Indigenous Community Wellness & Outreach Coordinator to support its Mobile Health Clinic. You will coordinate activities, build community relationships and help clients access health and social services across communities.

Reporting to the Integrated Care Manager, you will navigate care pathways, ensure continuity across clinics, and contribute to program planning while operating the clinic vehicle.

Qualifications

• Post-secondary education in health or social services or equivalent.
• Experience in community outreach, client navigation, health or social services.
• Knowledge of Indigenous health and social determinants of health.
• Strong relationship-building, communication, organization, coordination and problem-solving skills.
• Ability to work independently and in an interdisciplinary team.
• Experience with Indigenous communities preferred.
• Driver's license and willingness to travel; vehicle operations may be required.
• First Aid/CPR certification or willingness to obtain.
• Vulnerable Sector Police Check current or willingness to obtain.
• Willingness to participate in Indigenous Cultural Safety Training.

Responsibilities

• Operate the Mobile Health Clinic vehicle and assist with setup of supplies and equipment.

Salary: $56,699 to $70,874 per year

Southwest Ontario Aboriginal Health Access Centre (SOAHAC) is a diverse, dynamic, multiservice Indigenous health and wellness agency. We are currently seeking an Indigenous Community Wellness & Outreach Coordinator to join our interdisciplinary team and support SOAHAC’s Indigenous Mobile Health Clinic at our Cambridge site.

Reporting to the Integrated Care Manager, the Coordinator works alongside Nurse Practitioners, Physicians and other members of the interdisciplinary team to bring accessible, culturally safe and wholistic health and wellness services directly into communities.

As the consistent point of coordination for the Mobile Health Clinic, this role combines community outreach and client navigation with the coordination required to keep mobile clinic services organized and connected as clinical team members rotate through service delivery. The Coordinator builds community relationships, supports clients in navigating health and social services, coordinates follow-up and helps ensure continuity across clinics, providers and community partners.

Responsibilities

• Coordinate Mobile Health Clinic activities, including schedules, community locations, outreach, logistics and follow-up.
• Build and maintain respectful relationships with Indigenous individuals, families, communities and service partners.
• Support clients in navigating health, wellness, cultural, social and community services and facilitate appropriate referrals and connections.
• Coordinate non-clinical follow-up and help address barriers to accessing care and supports.
• Work collaboratively with Physicians, Nurse Practitioners and other SOAHAC programs to support coordinated and wholistic service delivery.
• Identify service gaps, emerging community needs and operational challenges and work collaboratively to support appropriate solutions.
• Maintain accurate and confidential documentation and contribute to program monitoring and service planning.
• Operate SOAHAC’s Mobile Health Clinic vehicle and support the safe transportation and setup of clinic supplies and equipment.

Requirements

• Post-secondary education in social services, health, Indigenous studies, community development, human services or a related field, or an equivalent combination of relevant education, training and experience.
• Experience in community outreach, client navigation, health or social services, Indigenous community services, service coordination or a related environment.
• Knowledge and understanding of Indigenous health and wellness, social determinants of health and barriers experienced by Indigenous individuals and communities in accessing services.
• Strong relationship-building, communication, organization, coordination and problem-solving skills.
• Ability to work independently and collaboratively within an interdisciplinary and community-based environment.
• Experience working with Indigenous individuals, families and/or communities is strongly preferred.
• A regulated health or social work designation is not required but considered an asset.
• Valid Ontario Driver’s “G” licence; clean driver’s abstract, as well as proof of personal auto insurance (must be insured for a minimum of three years and in good standing). Travel is a requirement of the position.
• Must meet SOAHAC’s requirements to operate organizational fleet vehicles and be able and willing to regularly operate the Mobile Health Clinic vehicle.
• First Aid/CPR certification required or willingness to obtain.
• Clean and current Vulnerable Sector Police Check as a condition of employment.
• Willingness to participate in Indigenous Cultural Safety Training, teachings and ceremonies.

This job may require additional responsibilities and duties as assigned by Southwest Ontario Aboriginal Health Access Centre.
