Job summary

Southwest Ontario Aboriginal Health Access Centre (SOAHAC) is seeking a Diabetes Nurse Educator to provide culturally safe, client-centered diabetes education to Indigenous adults with Type 2 diabetes. You will work with a multidisciplinary team to support self-management, improve health outcomes, and coordinate care through individual counselling, group education, and outreach.

This permanent, full-time role requires Ontario nursing registration, a minimum of three years in diabetes education,

Salary: $79,672 – $99,590 per year

Position Summary

The Diabetes Nurse Educator provides culturally safe, client-centered diabetes education and support to Indigenous adults living with Type 2 diabetes and pre-diabetes. Working as part of a multidisciplinary Diabetes Education Program team, the Nurse Educator supports clients in developing practical self-management skills, improving health outcomes, and accessing coordinated care.

The role includes individual counselling, group education, outreach activities, and participation in program monitoring and evaluation in alignment with Ministry-funded Diabetes Education Program requirements.

Responsibilities

Clinical Diabetes Education & Care

• Provide diabetes education and counselling to clients using evidence-based clinical practice guidelines
• Support clients with goal setting, self-management, medication understanding, lifestyle changes, and complication prevention
• Develop and monitor individualized care plans in collaboration with clients and care providers
• Maintain accurate and timely electronic medical documentation

Group Education & Health Promotion

• Plan and deliver group education sessions and health promotion activities focused on diabetes prevention and management
• Develop and adapt culturally appropriate educational materials
• Support community outreach initiatives and workshops

Program Support & Quality Improvement

• Participate in program evaluation, outcome tracking, and reporting activities
• Contribute to continuous quality improvement initiatives
• Collaborate with interdisciplinary team members to support coordinated care delivery

Qualifications & Experience

• Undergraduate degree in Nursing (BScN or equivalent)
• Current registration in good standing with the College of Nurses of Ontario
• Minimum three (3) years of relevant clinical experience in diabetes education or chronic disease management
• Demonstrated experience working in interdisciplinary care settings
• Knowledge of current diabetes clinical practice guidelines
• Strong communication, education, and counselling skills
• Certified Diabetes Educator (CDE) designation considered an asset
• Experience working with Indigenous communities and knowledge of Indigenous health perspectives considered an asset
• Experience with group facilitation and community-based education considered an asset
• Ability to travel locally for outreach activities
• Valid driver’s licence, access to a reliable vehicle, and appropriate insurance for work-related travel
• Successful completion of a Vulnerable Sector Police Record Check satisfactory to the organization.
• Proof of immunization and/or immunity in accordance with SOAHAC’s Occupational Health and Safety policies and applicable public health guidance.
