Job summary

A home health care provider in York Region is seeking a part-time Community-Based Personal Support Worker (PSW) to provide personal care and support to clients in their homes. Responsibilities include assisting clients with daily activities, meal preparation, and ensuring their safety. The ideal candidate must have PSW certification, a valid driver's license, and be able to work 12-hour shifts on weekends. Qualifications

• Must have a valid driver's license and reliable access to a car.
• Negative TB test completed within the last year.
• Immunization for measles, rubella, varicella, tetanus, and diphtheria required.

Responsibilities

• Provide personal care to clients as outlined in their care plans.
• Assist clients with transfers, ambulation & exercises.
• Complete records and documentation accurately and thoroughly.
