Job summary

Indwell in London, Ontario is seeking a Supportive Housing Coordinator to work with residents facing mental health and substance use challenges. You will develop and implement individual support plans, coordinate services, and participate in interdisciplinary teams to promote housing stability and wellbeing.

Qualifications

• Post secondary education in related field or equivalent.
• Experience working within a case management framework with individuals with serious mental illness, substance use, or other special needs.
• A warm, sensitive, and caring approach.
• Ability to maintain good interpersonal relationships and have a genuine interest in supporting people living with mental illness.
• Valid driver’s license (G class) with own vehicle.

Responsibilities

• Direct Support to Residents: build therapeutic relationships, coordinate and participate in case conferences, implement individual resident support plans, connect residents with resources, document thoroughly.
• Leadership: lead internal program development and coordinate team efforts; maintain relationships with community partners.
• Support, Direction, and Commitment to Interdisciplinary Team: work with interdisciplinary team to support housing stability, assist orientation of staff and volunteers, keep up with best practices.
• Health, Welfare and Safety Practices: follow safety protocols, attend trainings, ensure health and safety of tenants and staff.
